The age-related incidence is bimodal, usually with an early peak in infancy and childhood, a gradual rise in young adulthood, and a broader, sustained peak during the fifth to eighth decade of life. The expected early childhood peak is absent in Idaho's population. Instead, what is observed is an early middle-age peak in addition to the sustained peak during the fifth to eighth decade of life.
